Primary health care covers a broad range of health services, including diagnosis and treatment, health education, counselling, disease prevention and screening.A strong primary health care system is central to improving the health of all persons and reducing health inequalities between different groups. Our aim is to promote a more seamless patient journey across community, primary, and hospital sectors, greater use of primary and community care, and care being provided closer to the patient’s home.

Primary care is the day-to-day health care given by a health care provider. Typically this provider acts as the first contact and principal point of continuing care for patients within a health care system, and coordinates other specialist care that the patient may need. Patients commonly receive primary care from professionals such as a primary care physician (general practitioner or family physician), a nurse practitioner (adult-gerontology nurse practitioner, family nurse practitioner, or pediatric nurse practitioner), or a physician assistant. In some localities such a professional may be a registered nurse, a pharmacist, a clinical officer , or a Ayurvedic or other traditional medicine professional . Depending on the nature of the health condition, patients may then be referred for secondary or tertiary care. A Primary Care Team (PCT) is a team of health professionals who work closely together to meet the needs of the people living in the community. They provide a single point of contact to the health system.

These professionals include:

  • GP and Practice Nurse
  • Community Nursing Service-Public Health Nurse, Community Registered Nurse
  • Occupational Therapist
  • Physiotherapist
  • Home Help/support staff

The Primary Care Team members also link with other community-based disciplines to ensure all health and social needs are provided for: These include:

  • Speech and Language Therapy
  • Dieticians
  • Mental Health Services
  • Counsellor/Psychologist
  • Podiatry
  • Social work
  • Dental
  • Ophthalmic Services

Secondary Care

Health services provided by medical specialists and other health professionals who do not have first contact with patients are considered providers of secondary care. This includes acute care which is considered short-term treatment of a serious injury or period of illness, usually relatively urgent. Secondary health care can also refer to ongoing services not necessarily provided in the hospital, such as psychiatrists, physiotherapists and occupational therapists.

Our aim is to understand the organization of health practices, based on the interactions at the secondary care level, and to analyze how the actions and services at this level of care contribute to the development of best practice in health.

A qualitative approach, based in Grounded Theory. Data was obtained from individual interviews, with managers, health care professionals and health service users making up the sample group representing the secondary level of health care. The theoretical model was formulated based on four categories, analyzed based in the elements of the network modeling of health care theoretical framework.

Secondary Health Care is an intermediate level of health care, that is concerned with the provision of specific technical, therapeutic or diagnostic services. Specialist consultation procedures and hospital admissions fall into this category of care. These services are episodic and usually focused on a particular health problem.

We provide IT solutions both for individual hospitals and for regional networks that include multiple components such as hospitals, health care centres, private clinics, and occupational health care suppliers. Our easy-to-use, patient-centric, process-oriented Seconday Care solution for hospitals guides professionals in making the correct medical decisions while controlling operational costs.

Our solution covers all processes and workflows, facilitating the development of efficient and high-quality customer processes within hospital units and specialities including ambulatory clinics, inpatient wards, surgical theatres, and specialized supporting units such as radiology and laboratory. Information is always accessible and reliable wherever it is needed – allowing the right decisions to be made and the right actions to be taken.

Tertiary Care

Approach to health care in urban, rural areas is multidimensional. Improvement in health care services are interlinked with auxiliary services like public health care system, transport communication systems like road, railways, air-link for remote, inaccessible areas, modern communication system like tele-medicine, tele-link, satellite-link system etc. Hence improvement in auxiliary services improves health care system from rural to urban areas.

Role of private sectors apart from government sectors has tremendously increased in tertiary health care, human resources development, private-public partnership to cover a larger section of underprivileged to get high tech care at affordable cost. Presently, we have profitmaking private, charitable private, public sector, government health care delivery system, which should proactively look into social aspect, community based approach to health care delivery without any prejudice.

Sometimes, your patients will require the kind of complex consultative care that only a tertiary care facility can provide. In those cases, you may wish to make a referral to a tertiary care center. All referrals to plan tertiary care centers are reviewed on an individual basis. When we review these referrals, we mainly consider the member's medical needs and whether the requested services are available in the non-tertiary care network.

Tertiary care requires highly specialized equipment and expertise such as coronary artery bypass surgery, renal or hemodialysis, some plastic surgeries or neurosurgeries, severe burn treatments or any other very complex treatments or procedures. A small local hospital may not be able to provide these services and you may need to be transferred to a medical center that provides these highly specialized tertiary level services.

The institutions specialized in the treatment of cancer must have experienced and specialized personnel, a wealth of medical knowledge, devices manufactured with state of the art technology, modern medical methods and infrastructure systems that will allow coordination between specialists.

Quaternary Care

Quaternary Health services include all services dealing with the diagnosis and treatment of disease, or the promotion, maintenance and restoration of health. They include personal and non-personal health services. Improving access, coverage and quality of services depends on these key resources being available; on the ways services are organized and managed, and on incentives influencing providers and users.

Focus on accountability, quality outcomes and affordability have led to structural changes in the Quaternary Health care industry. As the thrust of Health care changes from fee-for-volume to fee-for-value, all players in the ecosystem must re-examine their models. Two factors critical for delivering cost effective and high quality care are a robust system to deliver Health care services and products, and a sustainable reimbursement model to cover the costs of care.

Depending on your health, you require different levels of care. If you are healthy or have a common illness such as a cold, you need help from a primary care physician.
